Vaccination waiversMost states allow parents to sign a waiver instead of having their child vaccinated before entering school. We have the right to choose this option for religious, medical or just plain don't believe in vaccinations. We have one of the more liberal state vaccination laws.
This form is legal in the state of Michigan, I prefer one of the other forms which don't make you feel like you are a criminal and bad parent when you sign it.
